CareHarmony and Ardent Health Services partner to implement AI-powered chronic care management program

Program helps patients overcome social determinants of health, increase clinical treatment adherence

NASHVILLE, Tenn. (Oct. 6, 2023) – CareHarmony, a leading AI-powered care coordination provider, has entered a strategic partnership with Ardent Health Services. CareHarmony’s human-driven, machine learning enhanced Chronic Care Management program will be deployed in Ardent facilities to bolster care delivery and support value-based care initiatives.  

Ardent is a leading provider of healthcare in communities across the country through its 30 hospitals and more than 200 sites of care with over 1,400 providers. CareHarmony’s innovative care coordination solutions are proven to improve patient care in a holistic manner that accounts for both the clinical and social aspects of health.

Following an initial launch at Ardent’s UT Health East Texas market, patient engagement in preventative care programs increased over 1,000% within three months. Over 165 providers across nearly 50 primary care and specialty locations are currently participating in the UT Health East Texas program. With positive outcomes from the initial pilot, Ardent is scaling its program with CareHarmony to other markets – including the recently launched program at Lovelace Health System.
